Patient Safety Incident Response Framework (PSIRF)

Children’s Hospice South West is committed to providing safe, compassionate, and effective care at all times. However, as with all health and care services, unexpected events may occasionally occur. As an organisation, we encourage an open culture of reporting, including near-miss incidents, to foster learning and continuous improvement in ensuring the safety and well-being of everyone who uses or visits our services.

About PSIRF

PSIRF is a key part of the NHS Patient Strategy, which aims to help continuously improve patient safety and build the foundation of a safe working culture and systems.

The PSIRF replaces the current Serious Incident Framework and will focus on and support the development of an effective patient safety incident response that integrates 4 key aims:

  • Compassionate engagement and involvement of those affected by patient safety incidents
  • Application of a range of system-based approaches to learning from patient safety incidents
  • Considered and proportionate responses to patient safety incidents
  • Supportive oversight focused on strengthening response system functioning and improvement
